Technology is a double-edged sword; it can either be a source of comfort or a cause for anxiety. Recently, many healthy individuals have started using smartwatches to monitor their vital signs, such as blood pressure and oxygen levels, among others. However, the continuous monitoring of their health status throughout the day can become a source of anxiety that burdens their thoughts and makes them more preoccupied with their health.
A number of doctors and specialists have confirmed that monitoring heart rate and other vital signs without real symptoms can, over time, become an unnecessary burden, as it may contribute to increased health anxiety and stir fears instead of achieving reassurance and mental comfort. Specialists point out that smartwatches were primarily designed to be supportive tools that help users track their health and daily activity, not to diagnose diseases or replace specialized medical examinations. Therefore, excessive reliance on their readings and the repeated monitoring of vital signs may lead to interpreting natural data changes as danger signals, creating a state of tension and constant doubt in some individuals. Experts emphasize that the true benefit of these devices lies in using them moderately and consulting specialists when actual symptoms or abnormal repeated readings appear, rather than worrying about continuously tracking numbers. Mental health is just as important as physical health, and any technological means should provide reassurance and improve the quality of life, not turn into a source of daily obsessions and fears. Amid the rapid development of health technologies, the responsibility remains on the user to achieve a balance between benefiting from the advantages of these modern tools and avoiding the trap of excessive monitoring, which may make a person a prisoner of numbers and measurements instead of living life normally and reassuringly.
Do not monitor your pulse
“Okaz” surveyed several doctors and specialists to understand the health and psychological effects associated with the increasing use of smartwatches and to learn about the accuracy of their readings, the limits of their benefits, and the potential risks of excessive reliance on them.
Cardiology consultant Dr. Khalid Al-Ghamdi explained to “Okaz” that smartwatches, according to the guidelines of heart associations, can be useful in diagnosing certain pulse disorders, such as tachycardia, atrial fibrillation, and bradycardia, especially in individuals who experience intermittent palpitations, as these devices help detect some pulse disorders that may be difficult to identify during traditional examinations.
“I have seen in some cases that a patient’s monitoring of their pulse has turned into daily anxiety, and I advise some patients not to monitor their pulse to alleviate the anxiety associated with using smartwatches.”
Al-Ghamdi continued: If a person notices that they are living in constant anxiety due to monitoring their pulse, it is better to consult a doctor about the changes they observe and to reduce the use of the watch unless necessary; because monitoring the pulse has become, in some cases, a source of anxiety.
Alerts leading to emergencies
Emergency medicine consultant Dr. Youssef Ajaj sees that emergency departments are indeed witnessing the presence of healthy individuals after receiving alerts from their smartwatches, such as “Your pulse is high,” or “Oxygen is low,” or notifications of a potential heart rhythm disturbance.
Dr. Ajaj explained to “Okaz” that some of these alerts may be important and warrant medical evaluation, while others may result from a transient or inaccurate reading that, due to excessive monitoring, turns into a real source of anxiety.
He indicated that it is worth heading to the emergency department when an abnormal reading coincides with concerning symptoms, such as chest pain, shortness of breath, fainting, or feeling faint, or when there is severe and persistent palpitations, or when abnormal alerts are repeated, especially in heart patients and those with risk factors.
He confirmed that smartwatches represent an important advancement in the health field and may contribute to the early detection of some health issues, but they are “not a doctor on the wrist,” and a healthy person should not turn into a patient who monitors their body’s numbers all day. He stated that health technology is designed to help us live healthier lives, not to live in anticipation of the next alarm.
Do not ignore alerts
Dr. Ajaj added that this phenomenon, from the perspective of working in emergency departments, has a dimension that goes beyond individual anxiety; attending the emergency department due to transient readings or alerts from smart devices, without concerning symptoms, adds an avoidable burden on departments that are already suffering from congestion, noting that every non-emergency visit consumes time and resources that may be needed by another patient in a more critical condition.
He emphasized that the responsibility lies not in downplaying the importance of these devices or ignoring their alerts, but in raising awareness about how to interpret them, knowing when to schedule a medical appointment, and when it is indeed necessary to go to the emergency department.
Reassuring the patient and assessing the condition
The family doctor is considered the first line of defense in the healthcare system, as family medicine consultant Dr. Fatima Houldar sees that clinics are increasingly facing cases related to what is known as “digital health anxiety,” where some patients continuously monitor vital signs, such as heart rate, blood pressure, oxygen levels, and other indicators, through smart devices and applications. This monitoring can be beneficial in some medical cases when used correctly and under medical supervision, but excessive monitoring may lead to increased anxiety and an excessive focus on natural changes in the body.
She told “Okaz” that a patient may interpret any slight increase or temporary change in one of the indicators as a sign of a serious illness, which increases tension and anxiety, affecting the quality of life, sleep, and daily activity.
Houldar pointed out that the role of the family doctor involves educating and reassuring the patient and comprehensively assessing the symptoms, in addition to guiding them on the correct use of digital health technologies, so that they serve as supportive tools for health rather than a continuous source of anxiety.
She confirmed that the goal is not to prevent the use of these technologies, but to promote their conscious and balanced use, linking their results to the correct medical context and the patient’s overall condition.
A source of wellness... not anxiety
Regarding the transformation of monitoring body symptoms into obsessive-compulsive behavior, senior clinical psychologist Dr. Jameel Al-Katib explained to “Okaz” that smartwatches and wearable devices started as modern tools that help individuals understand their bodies better; they monitor heart rate, sleep quality, activity level, oxygen, and some other health indicators. He noted that the paradox lies in the fact that technology designed to enhance wellness can become a constant source of anxiety for some healthy individuals.
He stated that it often begins with a simple desire for reassurance, then gradually develops into repeated monitoring of the body: “How many beats now? Why has the rate increased? Is this reading normal?”
Over time, a person becomes excessively alert to the slightest physical sensations, and the number displayed on the screen may become more important than their actual feelings about their body. Here, a well-known psychological cycle begins; checking provides temporary reassurance, but it does not end anxiety, and may even reinforce the need for further checking. Whenever a different reading or a transient acceleration in the pulse appears, the mind may fall into the trap of “catastrophic interpretation” or “magnification,” which are cognitive errors, treating a natural physiological change as a sign of a serious illness.
He added that the paradox increases when the anxiety itself stimulates the nervous system, causing the pulse to rise or palpitations to increase, which the watch then detects, and the person reads as evidence confirming their fears.
He said: “Thus the loop continues: fear, then monitoring, then physical change, then greater fear.” He emphasized that the problem is not with the technology itself, but with the relationship of the individual to it, explaining that smart devices become useful when they provide information that helps individuals make better health decisions, not when they cause them to live in a state of constant searching for illness.
He added: “Wellness does not mean that we monitor our bodies every minute, nor that we demand perfect numbers all the time. Sometimes, part of mental health is trusting the body and allowing the heart to beat without asking it every moment: What is your pulse now?”
Cases and paradoxes in emergencies
Emergency medicine consultant Dr. Youssef Ajaj recounts a case that visited the emergency department: a healthy young man who came in anxious, without experiencing chest pain or shortness of breath. After conducting a medical evaluation and an ECG, the results were reassuring, and it turned out that the reading that caused his anxiety appeared after physical exertion.
In another case, a person without any symptoms arrived at the emergency department after waking up to an alert from their smartwatch indicating a low heart rate during sleep, which is normal in some cases. After conducting a medical evaluation, the team found nothing indicating a problem that warranted concern.
Family medicine consultant Dr. Fatima Houldar recounts the case of a patient who came to the clinic due to his ongoing anxiety about an elevated heart rate.
She explained that the patient was excessively and repeatedly checking his pulse through the smartwatch, and as his anxiety and worry increased, he was shown elevated readings of his heart rate, which increased his fear and anxiety and led him to continue monitoring.
She added that after conducting a clinical examination and medical evaluation, it became clear that the patient was healthy and had no health issues, and that his heart rate was normal.
She pointed out that the patient was reassured and it was explained that the anxiety and stress resulting from continuous monitoring could cause a temporary increase in heart rate, and that excessive monitoring of vital signs through smart devices could increase anxiety instead of reassuring the patient.
