The dermatologist is no longer present in the minds of many merely as a specialist in diagnosing and treating skin diseases; rather, it has become associated for a segment of society with "Botox, fillers, and laser" and aesthetic enhancement procedures. This perception has been reinforced by the widespread proliferation of cosmetic clinics, the density of advertisements, and the presence of beauty content on social media platforms.
While patients visit dermatology clinics to treat various health issues and problems, others increasingly seek them out for aesthetic improvements and to address imperfections, burn scars, and pigmentation issues. As this aspect of medical practice expands, a question arises as to whether the growth of cosmetic services has altered the public's perception of the specialty or if the media focus on this aspect has overshadowed its broader therapeutic role.
The increasing prevalence of cosmetic procedures has linked the dermatology specialty more to aesthetic enhancement than to disease treatment, especially given the widespread visibility and following of cosmetic results and promotional advertisements.
Critics argue that this focus has made the cosmetic aspect seem like the primary function of the dermatologist, even though it represents only one facet of their specialty. Additionally, technological advancements and the diversity of available procedures have broadened the range of services offered by clinics, coinciding with the growing societal interest in external appearance, which has strengthened the presence of cosmetic practices in the public sphere, without necessarily indicating a decline in the therapeutic role of the specialty.
Eczema, Psoriasis, and Acne
Conversely, dermatology remains a medical specialty concerned with diagnosing a wide spectrum of diseases, treating them, monitoring their progression, and minimizing their complications. These diseases include eczema, psoriasis, acne, infections, allergies, hair loss, nail diseases, and pigmentation issues, in addition to autoimmune, genetic, and infectious diseases that may require precise examinations, treatment plans, and continuous follow-up.
The importance of this role extends beyond treating visible symptoms; it also encompasses identifying the causes of skin problems, managing chronic cases, and monitoring treatment responses, thereby maintaining skin health and limiting the progression and complications of diseases.
Citizens believe that some media platforms have contributed to cementing the image associated with cosmetic procedures by focusing on actions that attract the public and achieve wider dissemination, while there is less presence of content that introduces skin diseases, their diagnostic methods, and treatments, as well as the medical role that doctors play in dealing with various cases.
Here, the role of awareness is highlighted in educating the community about the breadth of the specialty and clarifying the difference between cosmetic procedures performed based on medical and professional foundations and practices that are predominantly commercially promoted, emphasizing that the appropriateness of any procedure remains linked to medical evaluation and the needs of the case.
The expansion of cosmetic services does not mean that the message of the dermatologist has changed; these procedures can be part of medical practice, especially when addressing the effects of burns, scars, and certain deformities and issues that affect patients' quality of life and self-confidence. However, this aspect should not overshadow the primary function of the specialty, which is diagnosing, treating, and monitoring diseases.
The challenge remains in restoring balance to the public's perception so that dermatology is not reduced to Botox, fillers, and lasers, nor is cosmetic enhancement viewed as the opposite of treatment; the specialty encompasses multiple fields, with responsible medical practice being its foundation, based on accurate diagnosis, appropriate treatment selection, and consideration for patient health and safety.
The Stereotype of "Dermatology"
Dr. Diana Al-Qadi, a dermatologist and cosmetic specialist, told "Okaz": The image of the dermatologist in the minds of some patients has become linked to procedures like Botox, fillers, and lasers, while sometimes their fundamental role in diagnosing and treating chronic, autoimmune, genetic, and infectious skin diseases is overlooked, emphasizing that diagnosis and treatment remain the foundation of dermatology.
Dr. Diana explained that technological advancements, the rising demand for aesthetic enhancement, the proliferation of social media, and the ease of showcasing results through images have all contributed to the increased presence of cosmetic procedures in dermatology. She pointed out that the results of cosmetic procedures are often easier to showcase in the media than the lengthy treatment journey of a chronic skin disease, which may take months or require continuous follow-up, making the cosmetic aspect more prominent in the overall image of the specialty.
She indicated that economic factors are present in this field, as some cosmetic procedures may yield financial returns for the clinic; however, the real challenge lies in ensuring that financial gain does not influence medical decisions, asserting that a doctor's recommendations should be based on the patient's needs and interests, not solely on profitability.
Imaginary and Idealistic Image
Regarding the impact of social media, Dr. Diana stated that it has raised awareness about cosmetic procedures, but at the same time, it has contributed to forming unrealistic expectations among some patients, especially with the presentation of idealized images without clarifying the risks or limits of results. She emphasized that the doctor's responsibility extends beyond merely performing the procedure; it includes providing accurate information, correcting misconceptions, and helping the patient make a realistic and safe decision that preserves their natural features and respects their desires.
She warned that an excessive focus on cosmetic procedures might, in some practices, consume time and resources; however, this cannot be generalized without evidence, noting that diseases like psoriasis, eczema, alopecia, and vitiligo require accurate diagnosis and continuous follow-up, and their priority should not diminish in the face of the allure of cosmetic services.
Dr. Diana stressed that possessing the latest devices or mastering injections does not replace the ability to examine the skin, diagnose diseases, and identify cases that require additional examinations or biopsies, affirming that clinical diagnosis remains the foundation of the specialty.
She pointed out that some patients may focus on treating pigmentation or hair loss without realizing that multiple underlying causes exist, clarifying that the doctor's role begins with identifying the cause, as merely improving appearance does not substitute for treating the underlying disease when present.
Risks of Quick Fixes
Dr. Diana warned of the risks associated with quick fixes performed without accurate diagnosis, as they may lead to delayed disease detection, inappropriate treatment choices, exacerbation of the problem, and complications, asserting that "the speed of the procedure should not precede the accuracy of the diagnosis."
Regarding unnecessary procedures, she confirmed that the doctor is responsible for evaluating the appropriateness and safety of the procedure, not merely executing the patient's request, explaining that refusing a procedure when its risks outweigh its expected benefits or when the patient's expectations are unrealistic represents an ethical and professional obligation.
She added that achieving a balance between profit and the profession's message requires adherence to transparency and sound medical justification, as success should not be measured solely by the number of procedures performed but by the quality of care, patient safety, and their trust in the doctor.
She emphasized the importance of starting the training of new doctors with a strong foundation in diagnosing and treating skin diseases, which should then be complemented by cosmetic training, as a doctor with a deep understanding of the skin and its diseases is better equipped to choose the appropriate procedure and avoid complications and manage them.
When Should We Not Perform It?
Dr. Diana clarified that the role of universities and training programs is to instill skills in differential diagnosis and treatment of complex and chronic cases, alongside providing scientific and safe cosmetic training, to produce a doctor capable of making the correct medical decision, not just mastering a specific procedure.
Concerning cosmetic medical advertisements, she stressed the need for accurate and non-misleading advertisements that clarify the limits of results and potential risks, protecting the patient and enabling them to make an informed decision.
She believes that the ideal model for a clinic that combines treatment and cosmetic procedures is one that begins with accurate medical evaluation, provides clear pathways for treating and monitoring skin diseases, alongside suitable cosmetic procedures for each patient, with a focus on safety, results, and managing complications.
Dr. Diana concluded her remarks by emphasizing that one should not have to choose between treatment and cosmetic procedures, as both are part of modern dermatology, but it should not be forgotten that the foundation of the specialty is understanding the skin and diagnosing and treating its diseases. She stated that the value of a dermatologist lies not only in the procedure they can perform but in their knowledge of when that procedure is appropriate and when it should not be performed.
Gap Due to Abundance
Dr. Mudar Ahmed Al-Qaim, a dermatologist and cosmetic laser specialist, believes that the dermatology specialty has witnessed development and transformation towards cosmetic diseases and procedures since the beginning of the 21st century, asserting that this is not merely a societal impression.
He explained that the field of cosmetology has emerged strongly in dermatology due to the remarkable results it has provided, in addition to the financial return, noting that financial gain plays a significant role in a dermatologist's inclination towards cosmetic procedures, without neglecting the important results achieved by cosmetic dermatology and its contribution to replacing some old treatments.
Dr. Al-Qaim confirmed that social media has significantly influenced perceptions of cosmetic dermatology due to the results presented by cosmetic doctors, indicating that health institutions and private clinics have also been affected by the increasing demand for aesthetic enhancement, which has reinforced their shift towards cosmetic dermatology.
He pointed out that the increased demand for cosmetic dermatology has not negatively impacted dermatology itself, as there are skin diseases that require a specialist dermatologist, affirming that clinical diagnosis remains the priority for dermatologists before transitioning to the cosmetic aspect.
Dr. Al-Qaim noted that there is a significant gap between cosmetic clinics due to the abundance of clinics; however, an experienced and skilled doctor plays the largest role in choosing the appropriate procedure and providing safe service to the patient.
Ethics Before Quick Profit
Regarding patients with vitiligo, psoriasis, and other skin diseases, Dr. Al-Qaim confirmed that they receive exceptional care from dermatologists, noting the existence of specialized centers for treating these diseases, adding that many patients focus more on the underlying cause than on appearance.
He emphasized the necessity for dermatologists and cosmetic specialists to be honest with patients and provide them with genuine advice, urging patients to consult a specialized and honest doctor to obtain appropriate treatment, explaining that a doctor may refuse a cosmetic procedure if it is unsuitable for the patient.
He indicated that achieving a balance between financial gain and cosmetic procedures is through choosing the right clinic and the right doctor, stressing the importance of universities and training centers in developing doctors' skills, alongside attending scientific conferences and seminars that contribute to professional development.
He affirmed that a doctor must possess ethical qualities before considering financial returns, noting that there are specialized medical clinics in the Kingdom that offer advanced medical and cosmetic services.
Serious Cancer Indicators
Dr. Najib Al-Sayed Al-Farnawani, a dermatologist, believes that dermatology clinics have become associated in the minds of many in recent years with Botox and filler injections, wrinkle removal, skin lightening, and laser treatments, to the extent that the cosmetic aspect seems to be the primary function of the dermatologist, at the expense of their original role in diagnosing and treating skin diseases and preventing their complications.
He explained that the problem is not with cosmetic dermatology itself, as it is an important branch that contributes to improving appearance, mental state, and quality of life when practiced according to scientific and ethical standards, but rather in its transformation from a part of the specialty to an activity that overshadows its other tasks, especially with the speed of cosmetic procedures and their higher financial returns compared to monitoring chronic and complex skin diseases.
He emphasized that the skin is the largest organ of the body and can be an early indicator of internal, autoimmune, hormonal, and infectious diseases; rashes may be linked to autoimmune diseases, persistent itching to liver or kidney diseases, hair loss to thyroid disorders or iron deficiency, while some changes in moles may be an early warning of skin cancer.
He pointed out that diseases such as psoriasis, eczema, vitiligo, severe acne, alopecia, and fungal and bacterial infections require accurate diagnosis, examinations, and follow-up, and can psychologically and socially affect the patient and their ability to study and work.
He noted that social media has contributed to establishing exaggerated beauty standards, with the spread of edited images, phone filters, and advertisements that link self-confidence to a specific appearance and flawless skin, which may drive some individuals towards cosmetic procedures in response to social pressure rather than medical necessity.
Do Not Pitting Us Against Each Other
Dr. Al-Farnawani indicated that the increasing focus on cosmetic procedures may also affect medical training and reduce attention to complex skin cases and specialized clinics for psoriasis, eczema, hair diseases, immunity, and skin tumors, while some patients may resort to quick fixes, such as using creams, antibiotics, or corticosteroids without diagnosis, which may delay disease detection or cause side effects. Dr. Al-Farnawani stressed the necessity of not placing therapeutic and cosmetic medicine in opposition, affirming that both can be combined while maintaining the strength of clinical education, supporting clinics for chronic diseases and specialized fields, and establishing regulations for medical advertisements to prevent exploiting people's fears or portraying cosmetic procedures as risk-free.
He concluded by emphasizing that skin health should precede its beautification, diagnosis should precede procedures, and the patient's needs should precede commercial considerations, and that the strength of the dermatology specialty is not measured by the number of laser devices or cosmetic injections but by its ability to combine science, treatment, beauty, and protecting the patient from disease and exploitation.
Beauty and the Appeal of Platforms
Naif Al-Humaidain, a lecturer and specialist in strategic communication and institutional media in the Department of Media at King Saud University, stated that the repetition of beauty content presented by celebrities and influencers, the allure of digital platforms, and the speed of interaction with them have contributed to the increased social acceptance of cosmetic procedures, to the extent that for some, it has transformed from a personal choice into a pressing standard that affects self-esteem due to constant comparison with idealized images.
He noted that the impact of content extends to families, especially younger demographics, pointing out that approximately 38 million surgical and non-surgical cosmetic procedures are expected globally in 2024, including around 154,000 procedures in Saudi Arabia, according to data from the International Society of Aesthetic Plastic Surgery.
Al-Humaidain explained that the expanding demand and competition have led some advertising campaigns to link cosmetic procedures with self-confidence and achieving the ideal image, which may reduce self-affirmation to external appearance at the expense of cognitive and value dimensions, warning against the mixing of medical education with commercial marketing and the transformation of cosmetic procedures into promotional commodities.
He stressed that the issue is not with cosmetic procedures, given their legitimate therapeutic, restorative, and elective aspects, but rather in prioritizing commercial considerations over professional standards, affirming the importance of the media's role in enhancing health awareness, enabling the public to distinguish between medical information and advertisements, establishing disclosure and transparency, and encouraging doctors to provide responsible content. He confirmed that the goal should be to limit the commodification of cosmetic procedures in the media, not to limit them, in a way that supports informed decisions and preserves professional values.
Ideal Beauty Standards
Dr. Omar Abdulaziz Al-Sha'shi Al-Zahrani, an international trainer and educational and family consultant, believes that the proliferation of beauty and skincare advertisements and body sculpting and skin whitening through social media necessitates a conscious family role to protect children from comparisons and dissatisfaction with their appearance, emphasizing the importance of dialogue and inclusion instead of prohibition and rigidity.
Al-Sha'shi told "Okaz" that self-confidence begins with children feeling accepted and loved within the family, away from ideal beauty standards, noting that enhancing self-esteem protects them from psychological pressures and marketing that links beauty to external appearance.
He stressed the need to cultivate critical awareness among children by discussing the credibility of images and advertisements, the benefiting parties, and the reliability of cosmetic products and procedures, and verifying the licenses of health practitioners, which helps them distinguish between reliable medical information and misleading commercial content.
He called for guiding children towards healthy practices, such as personal hygiene, adequate sleep, balanced nutrition, drinking water, and exercising, and encouraging them to seek information from trusted specialists, avoiding falling for celebrity advertisements that may promote unreliable products or promises.
He affirmed that family role models influence children's attitudes and behaviors towards appearance and beauty, indicating that a family based on love, dialogue, and awareness enhances their self-confidence and helps them benefit from medical advancements without falling victim to misleading marketing, instilling in them that a person's value is not limited to their external appearance.
How Did "Hives" Disappear from "Noor's" Hand?
Regarding audience experiences, Noor told "Okaz": "I suffered for a long time from hives on my hands, and I consulted several dermatologists. The treatments varied between creams, injections, and moisturizers, but the improvement was not as I had hoped."
She added: "One doctor referred me to a cosmetic clinic, and when I visited it, I found that the treatment cost was high. One night, while browsing TikTok, I came across a recipe related to hives, which I applied, and with the continued treatment prescribed by doctors and skin care, I noticed a significant improvement and the disappearance of hives from my hands."
She confirmed that her experience made her more hopeful alongside medical treatment, noting that the creams and skin care she used alongside her spiritual experience contributed, in her view, to improving her condition, saying: "Thank God for the healing and improvement I have received."
Drinking Water and Moisturizing the Skin
"Sara" says she suffered greatly from the effects of eczema, which left dark spots and clear deformities on her hands, shoulders, and face, explaining that some of these spots were large, deep, and visible to the eye.
She added: "Thank God my skin has improved significantly, and my skin tone has become better, even though I spent a lot during the treatment journey between dermatology and cosmetic clinics."
Between cosmetic procedures and dermatology clinics, Mohammed Nasser Al-Ghamdi stated that he suffered greatly from the effects of sunburn on his skin, explaining that he worked as an engineer and spent long hours outside under the sun, which caused rashes and clear marks on his shoulder, face, and hands.
He added that he received treatment for years in dermatology clinics, with treatments varying between creams, lasers, and moisturizers, but their benefits were often temporary.
He said: "I then turned to cosmetic clinics to treat and restore what remained of the burn marks, and thank God, my skin improved significantly, but I spent large amounts of money during the treatment journey."
He explained that he became more cautious about protecting himself from sun damage by using sunscreen before going out and increasing his water intake and skin moisturizing, affirming that his condition has become better than before despite the large amounts he spent in cosmetic clinics.
The Need for Balance Between Both Aspects
In conclusion, the opinions of specialists and patient experiences converge on the fact that aesthetic development does not contradict the therapeutic role of dermatology, but the challenge lies in maintaining balance between the two aspects, so that the allure of cosmetic procedures, their returns, and ease of marketing do not overshadow the primary function of the specialty. A dermatologist does not start their work with injections or lasers but begins with examining the patient, understanding their complaints, arriving at the correct diagnosis, and then selecting the appropriate treatment, which may include cosmetic procedures as part of the solution when needed. The most important criterion remains that diagnosis should precede the procedure, and the patient's interests should precede commercial considerations.
