حذّرت دراسة أمريكية حديثة من تزايد حالات سرطان القولون والمستقيم بين المراهقين والشباب، بعدما أظهرت أن غالبية المرضى الصغار الذين شملتهم الدراسة لم يُكتشف المرض لديهم إلا في مراحل متقدمة، وسط دعوات للأطباء إلى عدم تجاهل الأعراض المستمرة حتى لدى صغار السن.
ووفقاً لنتائج الدراسة التي عُرضت خلال المؤتمر والمعرض الوطني للأكاديمية الأمريكية لطب الأطفال لعام 2026 في سان دييغو، حلّل الباحثون السجلات الطبية لـ84 مريضاً تبلغ أعمارهم 21 عاماً أو أقل، شُخّصوا بسرطان القولون والمستقيم بين عامي 2012 و2023 في عدد من المراكز الطبية.
80% اكتُشف لديهم المرض في مراحل متقدمة
وأظهرت النتائج أن 51% من المرضى كانوا في المرحلة الثالثة عند التشخيص، و29% في المرحلة الرابعة، ما يعني أن 80% من الحالات التي شملتها الدراسة كانت قد وصلت إلى مراحل متقدمة.
وكان متوسط عمر المرضى عند التشخيص 16.5 عاماً، فيما عانى 74% منهم آلاماً في البطن أو أسفل الظهر، و45% تغيراً في عادات الأمعاء، و43% فقداناً للوزن، و38% نزيفاً من المستقيم.
تأخر التشخيص لأسابيع
وكشفت الدراسة عن تأخر ملحوظ في الوصول إلى التشخيص؛ إذ استمرت الأعراض لمدة وسطية بلغت 12 أسبوعاً قبل تشخيص المرض، فيما بلغ متوسط الفترة حتى إجراء تنظير القولون 13 أسبوعاً.
ويرى الباحثون أن ندرة سرطان القولون والمستقيم لدى صغار السن قد تؤدي إلى إرجاع أعراض مثل آلام البطن وتغير عادات الأمعاء وفقدان الوزن والنزيف إلى أسباب أكثر شيوعاً وأقل خطورة، ما قد يؤخر اكتشاف السرطان.
معظم المرضى بلا تاريخ عائلي
ومن النتائج اللافتة أن 20% فقط من المرضى كان لديهم قريب من الدرجة الأولى أو الثانية مصاب بسرطان القولون والمستقيم، ما يعني أن معظم الحالات لم يكن لديها تاريخ عائلي معروف للمرض.
كما أظهرت تحاليل الأورام انتشار خصائص مرضية عدوانية؛ إذ كانت الأورام ضعيفة التمايز لدى نحو 42% من الحالات، وظهرت الخصائص المخاطية لدى 39%، وخصائص خلايا «الخاتم» لدى 33%.
أكثر من ثلث المرضى عانوا عودة السرطان
وأظهرت المتابعة أن 36.9% من المرضى تعرضوا لانتكاسة وعودة المرض، بمتوسط بلغ نحو 11 شهراً، وكان نحو 97% ممن عانوا الانتكاسة قد شُخّصوا أصلاً في المرحلتين الثالثة أو الرابعة.
وبنهاية فترة الدراسة، توفي 38 مريضاً، أي 45.2% من المجموعة، بسبب سرطان القولون والمستقيم.
أعراض لا ينبغي تجاهلها
وقالت الباحثة إيريكا أرنولد، زميلة أبحاث جراحة الأطفال والمشاركة في الدراسة، إن سرطان القولون والمستقيم أصبح أكثر شيوعاً لدى المراهقين والشباب، وإنه عند ظهوره في هذه الأعمار غالباً ما يكون عدوانياً ويُكتشف في مراحل متأخرة، مع ارتفاع معدلات الانتكاسة والوفاة.
ودعا الباحثون الأطباء إلى وضع سرطان القولون والمستقيم ضمن الاحتمالات التشخيصية عند استمرار أعراض مثل آلام البطن أو أسفل الظهر، والنزيف من المستقيم، وفقدان الوزن غير المبرر، والتغير المستمر في عادات الأمعاء لدى المراهقين، خصوصاً عندما لا يوجد تفسير آخر واضح لهذه الأعراض.
وأكّدت الدراسة أهمية زيادة الوعي بالمرض لدى الفئات العمرية الصغيرة وتسريع التشخيص عند ظهور علامات تستدعي الفحص، في ظل المؤشرات المتزايدة على إصابة أشخاص أصغر سناً بسرطان القولون والمستقيم.
A recent American study has warned of the increasing cases of colon and rectal cancer among teenagers and young adults, showing that the majority of young patients included in the study were not diagnosed until the disease was at advanced stages, amid calls for doctors not to ignore persistent symptoms even in younger age groups.
According to the results of the study presented at the 2026 National Conference and Exhibition of the American Academy of Pediatrics in San Diego, researchers analyzed the medical records of 84 patients aged 21 years or younger who were diagnosed with colon and rectal cancer between 2012 and 2023 at several medical centers.
80% were diagnosed at advanced stages
The results showed that 51% of the patients were in stage three at diagnosis, and 29% were in stage four, meaning that 80% of the cases included in the study had reached advanced stages.
The average age of patients at diagnosis was 16.5 years, while 74% of them experienced abdominal or lower back pain, 45% had changes in bowel habits, 43% experienced weight loss, and 38% had rectal bleeding.
Diagnosis delayed by weeks
The study revealed a significant delay in reaching a diagnosis; symptoms persisted for an average of 12 weeks before the disease was diagnosed, while the average time until a colonoscopy was performed was 13 weeks.
Researchers believe that the rarity of colon and rectal cancer in younger individuals may lead to attributing symptoms such as abdominal pain, changes in bowel habits, weight loss, and bleeding to more common and less serious causes, which may delay cancer detection.
Most patients had no family history
Notably, only 20% of the patients had a first or second-degree relative with colon and rectal cancer, indicating that most cases did not have a known family history of the disease.
Additionally, tumor analyses showed aggressive pathological characteristics; tumors were poorly differentiated in about 42% of cases, mucinous characteristics were present in 39%, and "ring" cell characteristics were observed in 33%.
More than a third of patients experienced cancer recurrence
Follow-up showed that 36.9% of patients experienced a relapse and recurrence of the disease, with an average time of about 11 months. Approximately 97% of those who experienced a relapse had originally been diagnosed in stages three or four.
By the end of the study period, 38 patients, or 45.2% of the group, had died due to colon and rectal cancer.
Symptoms that should not be ignored
Researcher Erica Arnold, a pediatric surgery research fellow and co-author of the study, stated that colon and rectal cancer has become more common among teenagers and young adults, and when it appears at these ages, it is often aggressive and diagnosed at later stages, with higher rates of relapse and mortality.
Researchers urged doctors to consider colon and rectal cancer among the diagnostic possibilities when persistent symptoms such as abdominal or lower back pain, rectal bleeding, unexplained weight loss, and ongoing changes in bowel habits occur in teenagers, especially when there is no clear alternative explanation for these symptoms.
The study emphasized the importance of increasing awareness of the disease among younger age groups and expediting diagnosis when signs warrant examination, in light of the increasing indications of younger individuals being diagnosed with colon and rectal cancer.
